The Troop's collection of downloadable forms...

Here are the Class I and II Personal Health and Medical Record forms. This form is required for Scouts attending camps, Camporees or other programs. The Troop would like all Scouts to complete and return the Class I portion at the beginning of each school year for the Troop's records so that these records are current. The Class III form is also available. This form is needed for any Scout to attend a high adventure camp, other events requiring a physical examination or for adult Scouts (>40 yrs) attending camps.

The Troop Health form as PDF and as an Excel spreadsheet. It is VERY important that the Troop have a complete, current copy of this form for each Scout.

The Troop Outing permission slip as a Word form. Download the form, enter the specific information for your event and print. Also, please email me (Grady) a copy of the final product. I will convert to PDF and post on the web, linked to the event via the calendar. Here is a demonstration (Word/PDF) created using data from an earlier event.
